- Attached all documents with admission form, detail of required documents is given in admission form.
- For Doctor of Pharmacy, Submit your complete admission form in college campus or city office with entry test fee only (non-refundable).
- For Doctor of Physical Therapy and Pharmacy Technician Program, Submit your complete admission form in college campus or city office with admission fee and 1st instalment of your tuition fee (non-refundable).
